Warwick oral surgeons' office closed; inspectors found mishandled medications

WARWICK, R.I. -- The Health Department has shut down the oral surgery office of Dr. Mohammad Banki and Dr. Frank Paletta after finding improper handling of controlled substances.

Banki and Paletta are both licensed as physicians and dentists. The Health Department put no restrictions on their licenses, but ordered their 243 Jefferson Blvd. office closed until they improve security and record-keeping.

Inspectors from the Board of Dental Examiners found an array of violations, including: medications purchased from three companies not licensed in Rhode Island; an assistant who said he administered intravenous drugs; incorrect and inadequate documentation; misbranded and unlabeled medications; drugs kept in the same refrigerator as food and drink; controlled substances kept in an unlocked cabinet, and improper compounding of an anesthesia drug.
